Role Description
We are seeking a Quality Improvement Nurse to support clinical quality, patient safety, and continuous improvement across our home care and home health services. This role is ideal for a registered nurse who enjoys improving systems, using data to drive better outcomes, and partnering with teams to strengthen care delivery. This position is designed for growth. While not all skills are required on day one, we are looking for a nurse who is curious, improvement-minded, and eager to develop into a quality leadership role. This position is remote and offers you the flexibility of working from home.

Analyze quality, clinical, operational, and regulatory data to identify trends, patterns, and opportunities for improvement.

Review audit findings, incident reports, member complaints, grievances, NPS feedback, and regulatory outcomes to determine root causes.

Develop practical, actionable quality improvement plans and corrective action strategies based on data analysis.

Monitor the effectiveness of improvement initiatives and recommend adjustments as needed.

Create and maintain quality dashboards, trackers, workflows, and reporting tools to support continuous improvement efforts.

Collaborate with operational and clinical leadership to implement quality initiatives and drive measurable outcomes.

Present findings and recommendations in a clear, concise, and actionable manner.

Ensure compliance with organizational policies, accreditation standards, and regulatory requirements.

Prioritize multiple initiatives while managing projects independently with minimal supervision.

Serve as a resource for quality improvement methodologies and best practices across the organization.
Qualifications

Active Registered Nurse (RN) license required.

3β5 years of experience in Quality Improvement, Quality Management, or Performance Improvement within a healthcare setting.

Strong experience analyzing audit, incident, complaint, patient satisfaction (NPS), and regulatory data to identify trends and improvement opportunities.

Proven ability to develop corrective action plans and implement quality improvement initiatives.

Strong analytical, critical thinking, and problem-solving skills.

Ability to work independently, exercise sound judgment, and manage multiple priorities with minimal supervision.

Excellent communication and collaboration skills with the ability to partner effectively with clinical and operational leaders.

Proficient in Microsoft Office, particularly Excel.
Requirements

CPHQ certification (preferred).

Experience with CMS, NCQA, Joint Commission, or other regulatory/accreditation standards (preferred).

Familiarity with quality improvement methodologies such as Lean, Six Sigma, or Root Cause Analysis (RCA) (preferred).

Industry standards applicable to the state depending on work experience and level of expertise (preferred).
