Role Description
You will be one of the first prescribers in a new psychiatry service designed specifically for youth in and around foster care. You will have direct collaboration with a child and adolescent psychiatrist, a therapy team trained in evidence-based psychotherapy, and real influence over how the service works. Psychiatry launches in Virginia in early 2027 and will expand to other HNH states.
Key Responsibilities
-
Clinical care
-
Conduct 60-minute initial psychiatric evaluations and 30-minute medication management follow-ups by video.
-
Build treatment plans that pair medication with the evidence-based therapy each patient receives from their HNH therapist.
-
Prescribe and monitor psychotropic medications under HNH protocols, including baseline and follow-up labs, metabolic monitoring, and AIMS for antipsychotics.
-
Review and simplify complex medication regimens, which many of our patients arrive with after multiple placements and prescribers.
-
Prescribe electronically, including controlled substances through EPCS where permitted, and check state PDMPs as required.
-
Practice measurement-based care using serial screening instruments, and adjust treatment based on the data.
-
Assess risk at every visit and follow HNH safety protocols, escalating to the Lead Psychiatrist or emergency services when a patient needs a higher level of care.
-
Working within the child welfare system
-
Obtain informed consent for psychotropic medication from whoever holds legal authority (agency, court, or parent, depending on state and custody status) and obtain the youth’s assent.
-
Work within each state’s psychotropic medication oversight requirements for youth in foster care, including documentation and review processes.
-
Communicate with foster, kinship, and adoptive caregivers, caseworkers, and other members of the child’s team, within the limits of consent and privacy law.
-
Give caregivers clear, practical guidance on diagnoses, medications, side effects, and warning signs.
-
Protect continuity of care through placement changes, including medication reconciliation and timely records transfer.
-
Team and program building
-
Coordinate closely with each patient’s therapist and attend weekly interdisciplinary team meetings.
-
Complete documentation within 24 hours, with accurate coding.
-
Help build a new service by giving feedback on workflows, templates, and protocols, and taking part in quality improvement and peer review.
Who You’ll Care For
-
Children ages 6 and older, adolescents, and young adults through age 26 who have been in foster care.
-
Most have histories of complex trauma and multiple placements.
-
Patients who need a higher level of care are referred out and may return after stabilization.
How we work
-
Supervision: You report to the Founding Lead Psychiatrist, a board-certified child, adolescent, and adult psychiatrist, who provides weekly case review and on-call consultation and collaborative practice oversight where state law requires it.
-
Team: Psychiatry works alongside HNH therapists and Care Advocates. The full clinical team meets weekly; the medical team meets monthly.
-
Visit cadence: Psychiatry follow-up frequency is set by clinical need and can differ from therapy cadence.
-
Training: All HNH clinicians are trained in engaging youth with complex trauma histories and in our measurement-based care framework.
-
Setup: Fully virtual. HNH provides the EHR, e-prescribing, and telehealth platform.
