[Hiring] Physician Assistant/Nurse Practitioner - Mobile Integrated Health @South Shore Health

Role Description

Under the general guidance from the Executive Director of APCs with medical oversight from the MIH Physician leadership, the Physician Assistant or Nurse Practitioner will provide telehealth care to patients in the community. Rapidly growing service line adding provider coverage to meet the growing need in the community. The NP/PA will evaluate, diagnose and treat patients following written guidelines approved by the Department of Medicine.

Essential Functions

• Provide telehealth services to patients and community partners to operationalize COVID testing and/or discharge follow-ups after ED/Hospitalization for COVID symptoms.
• Telehealth evaluations and coordination for community-based testing and incoming phone requests from SNFs, ALFs, and Psychiatric Hospitals.
• Complete Provider to Provider screening and place orders for testing for symptomatic home-bound and at-risk populations.
• Document in SSH MIH Epic Code for provider to provider consultation.
• Coordinate with Registration to assure patient information is updated in Epic.
• Coordinate with EMS Dispatch to print lab labels and dispatch MIH/EMS to collect specimens.
• Telehealth follow-up with discharged patients, including reviewing Home Monitoring Telehealth Report and conducting follow-up visits.
• Document in SSH MIH Epic Code for Telehealth visits (telephone or video).
• Determine next course of action based on Telehealth visit outcomes.
• Consult with MIH paramedics and coordinate home visits as needed.
• Participate in continued learning and embrace technological advances that enhance communication and efficiency.

Qualifications

• ACLS (AHA) Advanced Cardiac Life Support Certification - American Heart Association (AHA).
• Physician Assistant - Board of Registration of Physician Assistants (Massachusetts).

Compensation

Compensation Pay Range: $100.00 - $120.00
