Role Description

This role will support efforts to improve pharmacy quality outcomes, with a primary focus on medication adherence, medication safety, and statin-related quality measures. The pharmacist will outreach to patients not meeting these care measures to support them in their medication journey. The pharmacist will also partner with providers and care teams on education and process improvement initiatives to address gaps in care. Based on training and prior experience, the position may also include providing comprehensive medication management (CMM).

Qualifications

• Bachelor's degree in Pharmacy OR PharmD from an accredited school of pharmacy
• 0 to 2 years of identified pharmacy experience

Requirements

• Completion of an accredited residency program (preferred)
• 2 to 5 years ambulatory care pharmacy experience including conducting assessment of patient medications, providing patient education, and performing care planning and consultation services (preferred)
• Licensed Pharmacist - MN Board of Pharmacy required upon hire if working in MN
• Licensed Pharmacist - WI Dept of Safety & Professional Services required upon hire if working in WI
• Pharmacist Board Certification preferred
• Valid Driver’s License required upon hire - MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment
