Role Description
Are you passionate about helping patients and providing exceptional customer service? Join our Enteral Nutrition Center team as a Patient Care Coordinator I (After-Hours Weekend) and play an important role in supporting patients who rely on life-sustaining nutrition therapy.
In this remote position, you will serve as a key point of contact for patients, caregivers, healthcare providers, and internal teams. You will help coordinate patient refill orders, verify documentation, maintain accurate records, and ensure patients receive the support they need throughout their care journey.
Key Responsibilities

Serve as the primary point of contact for patients, caregivers, referral sources, and branch locations regarding nutrition therapy orders and account inquiries.

Process monthly patient refill requests received through digital channels and inbound phone calls.

Review and verify patient demographics, insurance information, and order documentation to ensure accurate and timely shipment of supplies.

Update electronic patient records and maintain accurate account information.

Answer incoming calls, provide order status updates, and route inquiries to appropriate departments when needed.

Assist patients with refill requests, payment processing, and general account-related questions.

Investigate and resolve routine patient concerns while escalating complex issues to appropriate clinical or management staff.

Review patient accounts for outstanding balances and assist with payment collection activities as directed.

Receive and process credit card and other payment transactions accurately.

Support team members during high-volume periods and complex patient service situations.

Ensure a positive patient experience through professional, compassionate, and efficient service.
Qualifications

High School Diploma, GED, or equivalent.

Minimum of one year of customer service experience.

Minimum of one year of experience using Microsoft Office applications, including Outlook, Excel, and Word.

Strong communication, organizational, and customer service skills.

Ability to multitask and work effectively in a fast-paced environment.

Demonstrated problem-solving and decision-making abilities.

Ability to work independently in a remote environment while maintaining productivity and quality standards.
Preferred Qualifications

Experience in healthcare, pharmacy, home infusion, enteral nutrition, durable medical equipment (DME), or related healthcare support services.

Experience working with electronic medical records (EMR) or healthcare documentation systems.

Prior call center or patient support experience.

Schedule: Saturday 6:00 AM - 4:30 PM CST, Sunday 6:00 AM - 4:30 PM CST

20 hours per week (possible PRN hours to cover shifts available as needed up to 30 hours weekly)
Pay Range
The typical pay range for this role is: $17.00 - $25.65. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors. This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
