Role Description
Join the Upstream Rehabilitation team as a Patient Access Manager, where youโll play a key role in delivering meaningful patient experiences. This remote leadership role drives patient access performance across assigned clinics by leading a team of remote Patient Care Coordinators and partnering closely with Operations leadership to monitor access metrics and ensure consistent execution of referral management, scheduling support, insurance verification, and coverage. Your work will directly support Upstreamโs mission, vision, and values.

Lead and develop a territory-aligned team of remote Patient Care Coordinators (PCC-Rs), ensuring strong performance, accountability, and engagement.

Manage and allocate PCC-R coverage to support clinic volume demands, including PTO, vacancies, growth needs, and high-volume periods.

Oversee and ensure consistent execution of patient access workflows, including referrals, scheduling, benefits verification, authorizations, and EMR documentation.

Monitor territory access metrics (e.g., referral aging, scheduling lag, arrival rates), identify performance trends, and implement corrective action plans when standards are not met.

Partner with Clinic Directors, Vice Presidents of Operations, and Regional VPs to support field PCC performance and improve access outcomes.

Serve as the territory subject matter expert for patient access operations, providing visibility into performance metrics and ensuring consistent service delivery across clinics.

Qualifications

High school diploma or equivalent.

4+ years of experience in healthcare patient access or front-office operations.

2+ years of direct people leadership experience.

Experience leading and coaching remote teams, with accountability for performance management and corrective action.

Ability to analyze performance metrics, identify operational issues, and develop actionable improvement plans.

Strong cross-functional communication skills, partnering effectively with field and operations leaders.

Proven ability to manage workforce allocation, coverage planning, and workflow compliance.

Skilled at translating territory-level priorities into clear, structured daily execution for teams.
Requirements

5+ years of experience in a healthcare/medical practice setting (Nice to Haves).

Multi-site or territory-level oversight experience (Nice to Haves).
