[Hiring] Part Time Teletriage Nurse Practitioner/Physician Assistant @Eventus WholeHealth Careers

Role Description

Eventus WholeHealth delivers an integrated model of care to adults who reside in a variety of settings including but not limited to skilled nursing facilities, assisted living facilities, and independent living facilities. Teletriage provides telehealth and triage coverage for facilities across 5 states. Eventus WholeHealth Teletriage providers deliver medical services to patients at assigned contract sites according to site-specific protocols.

Shift: Hours: 10a-2p M-F 20 hours/week

NC License Needed.

Essential Duties and Responsibilities:

• Provide primary-care and acute psychiatry services in accordance with Eventus site-specific protocols.
• Perform other duties as assigned by Eventus Management.
• Manage facility and partner relationships, organization, patient consents, CCM, and billing.
• Utilize clinical judgment to triage needs and provide guidance, treatments, and care.
• Provide triage services per established protocol policy.
• Confirm treatment and telehealth consents are signed prior to telehealth visits.
• Follow Eventus telehealth visit etiquette protocols.
• Include family members in the visit, as appropriate.
• Complete documentation and sign all orders specific for the telehealth visit immediately after.
• Manage narcotic prescriptions per Eventus RX Policy.
• Notify the patient’s regular provider of visit and triage treatment plan.
• Establish and maintain open communication with facility staff and administration.
• Provide verbal and/or written instruction regarding medications and caregiver information.
• Collect and document all required data on the same day.
• Adhere to Eventus standards, policies, and procedures.
• Maintain professionalism and avoid gossip or negative comments.
• Maintain patient confidentiality and comply with HIPAA rules and regulations.
• Maintain multiple practice licenses and comply with state regulations.
• Full attention is required for the entire shift; this is not an on-call position.

Qualifications

• Master’s degree in Nursing from an accredited institution and a current and valid license to practice advanced practice nursing issued by the state.
• All certifications necessary to perform responsibilities must be current and valid.
• Specialization in geriatric, family, or internal medicine desired.
• ALF or SNF experience desired.
• Licensed in NC to start; state licenses required within 4 weeks of hire.
• OR graduated from a physician assistant educational program accredited by a National Commission.
• Passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
• Licensed in NC; state license required within 4 weeks of hire.
