[Hiring] Nurse Practitioner - Certified Wound Care @Wound Company Provider Group

Role Description

The Nurse Practitioner will be responsible for providing patient consultation, evaluation, and management via telehealth, virtual care, or digital monitoring modalities for patients with incisions, wounds, ostomies, and related skin disorders.

• Provide virtual clinical support across The Wound Company’s programs and geographic markets based on patient and program needs.
• Manage patient volume and workload expectations based on program needs, including scheduled telehealth visits, virtual clinical review, patient follow-up, care coordination, documentation, and clinical support activities.
• Support enrolled patients through assessment, treatment planning, follow-up, patient education, care coordination, and monitoring of clinical progress.
• Review available clinical information to support appropriate clinical decision-making.
• Document services to support the appropriate level of billing consistent with CMS guidelines.
• Complete all billable and non-billable service documentation within one (1) business day.
• Use judgment and critical-thinking skills to assess, diagnose, and identify appropriate outcomes and interventions.
• Prescribe wound, ostomy, and incision care within scope of practice.
• Collaborate with Clinical Support Coordinators and other members of the clinical team to coordinate care.
• Facilitate and coordinate care with other providers through the EMR and approved digital communication modalities.
• Communicate respectfully, clearly, and concisely with patients and team members.
• Support field-based Nurse Practitioners and other clinical team members with patient-specific clinical questions.
• Obtain additional state licensure when requested by The Wound Company.
• Evaluate processes, policies, procedures, and protocols for patient care.
• Participate in program development of the Nurse Practitioner program.
• Provide leadership and clinical input in quality-improvement initiatives.
• Maintain current wound-care certification and Nurse Practitioner licensure/certification.
• Uphold company policy for HIPAA compliance.
• Maintain a private and appropriate workspace for virtual clinical responsibilities.
• Complete other duties as assigned.

Qualifications

• Master’s degree in Nursing from an accredited nursing program.
• Licensed as a Registered Nurse by the State Board of Nursing (SBON).
• Board certified and licensed as a Nurse Practitioner in the state(s) where the Nurse Practitioner practices.
• Completion of a WOCN Society-accredited educational program and current certification by the WOCNCB in wound care (preferred).
• Alternative certifications: WCC or CWS.

Requirements

• Minimum of one (1) year of experience in a wound-related Nurse Practitioner role.
• Three (3) years of experience as a CWCN, CWON, or CWOCN (or equivalent experience).
• Five (5) years of experience as a WCC or CWS (or equivalent experience).
