Role Description
We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. In this role, you will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension.

Oversee patients, manage clinical escalations, and collaborate closely with the patientβs physician or provider to optimize treatment plans.

Adjust medications and determine the appropriate level of care.

Requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams.

This position is fully remote but highly collaborative, with clear expectations for productivity, responsiveness, and quality.
Qualifications

Active Compact Multi-State RN license.

Active NP licensure in CA, WA, or AZ.

5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D).

3+ years of cardiology experience with a focus on heart failure management and guideline-directed medical therapy (GDMT).

Active ANCC or AANP certification.

Masterβs Degree as a Nurse Practitioner.

Home office setup with consistently stable Wi-Fi.

Experience in a CHF bridge clinic or T2D clinic, a startup environment, and working with remote patient monitoring technology is preferred.

Deliver high-quality, evidence-based care by following established clinical protocols and workflows.

Initiate and titrate guideline-directed medical therapy (GDMT) while collaborating closely with care teams.

Respond promptly to clinical escalations generated through remote patient monitoring.

Provide both synchronous and asynchronous patient care.

Deliver safe, efficient, high-quality care while meeting established productivity and documentation expectations.

Adapt quickly to evolving workflows, software enhancements, and operational processes.

Use multiple technology platforms confidently and provide feedback to improve clinical operations.

Collaborate across clinical and operational teams to improve patient care, efficiency, and scalability.
