Role Description

The Program of All-Inclusive Care for the Elderly (PACE) Level of Care (LOC) Assessment Unit is seeking a Nurse Evaluator (NE) II to perform the most difficult and sensitive work within the unit to assess eligibility into the program. Under the general direction of the NE III, the NE II will:

• Review, validate, and document eligibility into PACE.
• Evaluate LOC determinations.
• Develop person-centered service plans.
• Integrate long-term services and supports for current and prospective PACE participants.

The incumbent must establish and maintain a working knowledge of the division's programs and policies as well as be able to respond to stakeholder and public sensitivities. The work performed is vital for the Californians we service, so join us to make a difference.

Qualifications

• CA Registered Nurse License required.
