Role Description
Weβre hiring a Migraine Care Navigator to serve as the primary point of contact for patients throughout their healthcare journey. This role is responsible for managing patient onboarding, coordinating appointments, supporting follow-up care, and ensuring accurate communication between patients, clinicians, and administrative teams. You will play a key role in maintaining a seamless and compassionate patient experience while helping improve operational efficiency across the organization.

Core Responsibilities

Conduct initial patient intake assessments and gather relevant health and symptom information.

Guide new patients through onboarding and explain treatment pathways clearly and compassionately.

Coordinate with providers to ensure accurate documentation of patient records and care plans.

Maintain organized and compliant patient intake documentation.

Patient Care & Support β 30%

Manage and schedule patient appointments to minimize delays and improve continuity of care.

Follow up with patients after appointments to monitor progress and care adherence.

Assist patients with rescheduling requests and appointment coordination.

Provide educational resources and respond to patient inquiries while escalating complex concerns to clinical teams.

Coordination & Quality Improvement β 25%

Collaborate with medical, billing, and administrative teams to maintain accurate patient records.

Support billing and insurance workflows, including prior authorizations and claims follow-ups.

Collect patient feedback and identify opportunities for operational improvements.

Assist in maintaining efficient communication between departments and care teams.

Operational Support & Communication β 15%

Resolve scheduling, documentation, and coordination issues in a timely manner.

Track assigned patient cases and ensure follow-ups are completed according to timelines.

Support SOP improvements that enhance communication and patient experience.

Qualifications

2+ years of experience in healthcare operations, patient coordination, clinical support, or care navigation roles.

Strong understanding of patient intake, appointment scheduling, and healthcare administrative workflows.

Experience supporting insurance verification, prior authorizations, or billing coordination processes.

Excellent verbal and written English communication skills.

Strong organizational skills with high attention to detail and accuracy.

Ability to manage multiple patient cases and priorities simultaneously.

Comfortable working independently in a fully remote, fast-paced environment.

Strong empathy and patient-centered communication approach.

Reliable internet connection and ability to work PST hours consistently.
