Role Description

Iconic Care Management is seeking a compassionate, organized, and self-motivated Remote Medical Assistant to support our growing Principal Care Management (PCM) program. This position plays a vital role in helping patients with complex chronic conditions receive ongoing care coordination, education, and support between provider visits.

• The ideal candidate is experienced in patient communication, care coordination, documentation, and working independently in a remote environment.
• This role requires excellent customer service skills, attention to detail, and the ability to build strong relationships with patients, providers, and caregivers.
• This role is best suited for a self-motivated and confident Medical Assistant who thrives in an independent setting.

Qualifications

• Medical Assistant Certification
• Minimum 5 years of medical office, care coordination, or chronic care management experience
• Strong computer skills and proficiency with EHR systems
• Excellent verbal and written communication skills
• Ability to work independently in a remote setting
• Strong organizational and time-management skills
• Knowledge of HIPAA regulations and patient privacy requirements
