[Hiring] MDS Coordinator @Hired

Role Description

The MDS Coordinator works in conjunction with physicians, other nursing staff, residents, families, and other facility departments to assure that the resident assessment tool, resident care plan, and interdisciplinary team meeting are completed according to all Federal/State regulations and according to facility policy/procedure in order to provide optimum individualized nursing care for the residents of SNF service.

Qualifications

• Minimum Data Set (MDS) Coordinator experience
• 1 year verifiable experience as a licensed nurse providing care and services in a skilled nursing or long-term care setting (preferred)
• Previous experience in MDS (preferred)
• Nursing Degree from an accredited college or university (required)
• RN/LVN California License (required)
• Current CPR (required)
• MDS certification (preferred)
• Current RN California License (preferred)

Requirements

• Must be a skilled communicator, director, and motivator
• Able to organize and prioritize many tasks effectively
• Working the floor and performing daily RN duties

• Contract length: 13 weeks
• Shifts: Days, 8 Hours
• Weekly Schedule: 40 Hours
