Role Description
Guidehouse is seeking an experienced healthcare consulting professional with post-acute care consulting experience. The candidate must have hands-on prior clinical and operations management experience in multiple post-acute settings including but not limited to home care, hospice, skilled nursing facilities, virtual/remote care, and/or inpatient rehabilitation, as well as be familiar with inpatient transitions of care issues. Experience in developing post-acute network strategies and value-based reimbursement models (e.g., bundled payments, MSSP, ACOs) is also desired.

Understand post-acute care workflows and be able to perform operational process assessments.

Articulate key process issues in the continuum of care, and how delays in transitioning to post-acute settings can impact acute care environments.

Identify process opportunities in discharge planning from the hospital setting to post-acute services, including assessment processes to streamline discharge from post-acute services to the community/home.

Redesign various post-acute programs and services including organizational structure, job descriptions, development of teams, policies, and processes.

Manage client engagements and day-to-day relationships to timeline, scope, and budget.

Oversee Senior Consultants and Consultants.

Draft client deliverables, including synthesizing the strategic, financial, and operational implications of analyses and recommended changes.

Assist with data acquisition, integration, and analysis to support a wide range of payer and provider analytics including financial analysis, clinical, and operational benchmarking.
Qualifications

A Bachelor’s degree from an accredited university, preferably in a clinical area such as therapy or nursing.

Current licensure related to degree obtained.

5 or more years of experience in post-acute clinical operations, transitions of care related work, and/or throughput experience in a provider setting, some of which is at the management level.

5 or more years of healthcare consulting firm experience and/or a combination of industry and consulting experience.

Knowledge of current trends including the impact of COVID-19 upon post-acute care, strategic positioning of post-acute programs and services, and implications of healthcare reform initiatives (e.g., payment reform).

Proficient in Excel, Word, and PowerPoint skills, and ability to independently produce documents or aptitude to develop this level of skill within a defined period of time.

Flexible, positive, clear interpersonal and communication skills with the ability to facilitate the exchange of information within all levels of GH as well as a provider-based client organization.

Analytical skills related to independently interpreting, preparing, and analyzing budget and workload data, LOS information, post-acute performance analysis, comparisons with relevant federal and/or other performance-based data.

Ability to develop and monitor relevant performance metrics at all levels.

Knowledge of, and familiarity with, relevant IT solutions and databases.

Knowledge of and ability to ensure and promote consistent, quality-oriented, national leading practices in all clinical operations recommendations.

Capacity to adapt and lead others to understand and accept values, strategies, goals, and plans in response to changing business conditions.

Ability and willingness to travel as much as necessary.
Requirements

Graduate degree.

Additional licensure, such as a Health Facility Administrator’s license.

Previous strategy consulting experience.

Demonstrates understanding of post-acute care operations; able to perform operational assessments.

Demonstrates understanding of strategic positioning of post-acute programs and services, including implications of healthcare reform initiatives (e.g., payment reform).

Understands post-acute reimbursement methodology.
-
Ability to articulate an understanding of the continuum of care, including acute, post-acute, and community-based services.

Ability to assess and recommend best practices related to post-acute venues of care.

Experience in transitions of care work, addressing prevention of readmissions, and care coordination.

Track record of expense reduction assessments and/or implementations and/or improvements in capacity/throughput for care delivery settings.

Assessment of provider organizations facing capacity and throughput issues.

Demonstrates understanding of discharge planning from the hospital setting to post-acute services, including pre-admission assessment processes.

Involvement in redesign of post-acute programs and services including organizational structure, job descriptions, development of teams, and policies and processes for performance.

Experience in redesigning post-acute clinical operations and change management.
-
Experience related to readmission prevention strategies within the post-acute environment.

Experience in managing projects, ideally as a consultant.

Experience with and understanding of continuum care management.
