[Hiring] LPN/LVN for RPM @RPM Healthcare

Role Description

As a bilingual (Spanish) speaking RPM 365 Care Coach, you’ll engage patients, track and analyze their data, provide education, and encourage adherence to their care plans.

• Serve as the primary contact for patients on the RPM365 platform.
• Assist with device setup and app connectivity.
• Engage with patients to ensure adherence to care plans.
• Schedule monthly check-ins and monitor patient data.
• Educate patients through shared resources.
• Notify healthcare providers of critical alerts.
• Attend virtual team meetings for updates.

Qualifications

• Active nursing license.
• Bilingual, Fluent in Spanish.
• Knowledge of chronic conditions and related monitoring devices.
• Knowledge of remote patient monitoring CPT code qualifications.
• Proficiency with technology, computer platforms, and smartphones.
• Strong verbal/written communication skills (English and Spanish).
• Comfortable with patient outreach and engagement (English and Spanish).
• Highly organized, detail-oriented, and team-focused.
• Independently motivated.
• Comfortable spending the majority of your workday engaging with patients over the phone providing support, education, and guidance.

Requirements

• Experience in telehealth, remote patient monitoring, education, and/or customer service (preferred).
• Experience in weight management coaching (preferred).
• Additional language skills are a plus (preferred).

Benefits

• Salary: $45,000-$49,000 (DOE) + performance bonuses.
• Schedule: Mon-Fri, 9:00 AM - 5:00 PM, EST (Remote).
