How you'll contribute:

  • Drive Multimodal Recovery Engagement: Utilize multi-platform technology (video, secure messaging, digital recovery content, and mobile tools) to maintain persistent, high-frequency outreach and re-engage members experiencing ambivalence or relapse risks.
  • Manage Panel Registry & Infrastructure: Actively participate in interdisciplinary clinical huddles, maintain registry data integrity, and track clinical touchpoints (up to 24 structured encounters) to optimize panel outcomes and ensure seamless care coordination.
  • Coordinate Medical & Community Wrap-Around Supports: Partner with external MAT/MOUD providers, medical teams, and community recovery organizations to address members' holistic medical, physical health, and health-related social needs.
  • Collaborate with the member, family, and healthcare providers to develop an individualized plan of care.
  • Document all member assessments, care plans, and referrals provided. Prepare and maintain all required records, reports, paperwork, and member’s diagnostic records, maintaining confidentiality of members at all times.
