[Hiring] Licensed Practical Nurse - Nurse Navigator @Geisinger

Role Description

Will be a Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients. The Nurse Navigator, functioning in the spirit of teamwork, is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena. Assists other team members on the unit. The nurse navigator role provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.

Qualifications

• Graduate from Specialty Training Program-Nursing (Required)
• Minimum of 2 years-Nursing (Required)
• Licensed Practical Nurse - Default Issuing Body
• Proficient computer skills
• Proficient with Microsoft products such as Word, Excel, PowerPoint, SharePoint, Windows, & TEAMs

Requirements

• Day/Evening position (40hrs/week)
• Hours will fall between 7:00 a.m. and 11:30 p.m.
• Shifts may be 12, 8, 6, or 4 hours in length
• All positions require working every other weekend
• Weekend shifts are 12 hours in length
• All LPNs work at least one major and one minor holiday per year
• Holiday shifts are 12 hours
• Less than 25% travel may be required for orientation/training, meetings, or other business needs
