Role Description

The Intake/Information Coordinator is responsible for coordinating referral sources to facilitate the admission of the patient to home care services. This position is responsible for managing administrative aspects of the patient intake process, including:

• Establishing and maintaining positive relationships with customers and referral sources
• Responding to customer requests and concerns
• Managing the insurance verification and authorization process
• Providing information to potential agency patients and referral sources regarding home health services in compliance with organization policies and applicable laws and regulations

Schedule:

• Every Saturday & Sunday; 8:30am - 5:00pm

Location:

• Remote; must live in the Bay Area

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Directing daily patient referral and intake operations, including implementation of intake policies
• Assuming responsibility for assessing initial patient needs for all disciplines
• Scheduling visits for field staff
• Ensuring patient admissions are according to state, federal and Joint Commission referral/intake regulatory requirements
• Maintaining current documentation in computerized clinical records according to agency policies and applicable laws and regulations

Qualifications

• High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience
• Previous experience performing clerical/administrative tasks in a healthcare setting or related position preferred

Benefits

• Pay Range: $25.00 - $35.00 hourly
