Role Description

We are seeking a compassionate Licensed Vocational Nurse to join our care team, whose responsibility is to provide quality care to our patients. This includes:

• Assessing patient's medical conditions
• Maintaining patient's comfort
• Meeting the needs of individuals within their home environments
• Providing guidance and support to their loved ones

Under the direction of a Registered Nurse, the Licensed Vocational Nurse is responsible for providing skilled nursing and direct patient care to assigned patients. Responsibilities include:

• Following the plan of care
• Providing treatments
• Working collaboratively with team members
• Conducting home visits for patients (Travel required for patient visits)
• Ability to work with terminally ill patients and their families
• Demonstrating competency in performance of skilled nursing functions
• Good communication with physician, caregivers, family, and hospice team members
• Using equipment and supplies effectively and efficiently
• Providing accurate and timely patient documentation (Charting)
• Providing appropriate pain/symptom management
• Evaluating patient’s response to treatments/medications
• Performing job compliance with agency policies, procedures and applicable laws
• Participating in quality assessment performance improvement teams and activities, including professional growth and development

Qualifications

• Must have valid/current California Licensed Vocational Nursing License
• Must have at least 1 year of recent LVN experience in home health, hospice, palliative care, acute care &/or skilled nursing
• Current CPR/BLS
• Current CA driver’s license with proof of insurance, reliable transportation and willingness to drive to patient's location
• Must have compassion, patience, be honest and dependable
