Role Description

Daily reconciliation of Emergency Department, Urgent Care and/or Observation logs to patient charts to initiate coding and billing.

Minimum daily chart production should be a minimum of 85-100 charts through manual pull or processing of stray and deficient charts.

Cross trained to work at least 1 or 2 other sites.

Utilize various methods of log and chart acquisition to include EMR, SFTP, DFS, scanning and feed sources.

Ensure log contains all required elements: facility name, patient name, account number, dos, MRN and CSN (where applicable) are configured into proper Excel format.

Thorough analysis of entire chart following current departmental policy and procedures of all required documents and complete provider documentation.

Once chart batch is submitted via Chart Receiver, confirm receipt of charts in Kofax ensuring correct batch properties, legibility and quality documents.

Accurate identification and assignment of deficient and non-bill charts ensuring these charts are not submitted until all requirements are met.

Research and resolve all outstanding strays, EDRC Exceptions and EDRC and quality deficient charts minimum twice a week.

Acquire, reconcile, analyze Inpatient Log and charts, and submit monthly via Kofax while maintaining the 10th of the month deadline.

Submit all logs and charts within 3–4-day deadline from dos or facility specific deadline using Chart Portal and Chart Receiver.

Assist Manager with offshore audits, non-bill audits, or other projects as needed.

Communicate with providers regarding incomplete charts by face-to-face meeting, electronically via EMR, telephone, My SCP App, or email.

Meet HiOps Departmental date of service chart submission lag time goal and monthly chart deadlines.

Accurately report daily production data into Jot Form program.

Maintain compliance with all SCP-Health Compliance policies and HIPAA guidelines at all times.

Ensure all SCP Health mail received at the facility is collected and sent to Dallas office weekly if HI-C onsite.

Function as onsite SCP Health representative for the providers and hospital staff; comply with hospital policy if onsite (where applicable).

Assist in additional duties as assigned.
Qualifications

Detail oriented, ability to organize tasks and set priorities.

Basic knowledge of Microsoft Office™ including Microsoft Excel™, Microsoft Word™ and Microsoft Outlook™.

Professional attitude and appearance to be able to communicate with hospital staff and providers.
Requirements

High school diploma.
-
0-2 years of experience in Medical Records.

Experience in Medical Records, HIM, Healthcare Clinic or Physician Office preferred but not required.
