Role Description
The Health Coach is a non-clinical member of the Heart Failure care team, supporting patients through education, behavior change, and engagement within Novocardiaโs value-based care model. This role partners closely with clinical team members to empower patients to better understand and manage their heart failure through prevention-focused, patient-centered coaching. The Health Coach plays a key role in improving patient experience, engagement, and self-management while maintaining clear boundaries from clinical decision-making and care delivery.
Key Responsibilities

Patient Education & Engagement

Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management.

Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques.

Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation.

Care Team Collaboration

Partner with the clinical care team to support patient care plans and escalate concerns appropriately.

Provide timely updates and documentation related to patient engagement and progress.

Participate in interdisciplinary team meetings.

Patient Experience & Communication

Respond to patient communications in a timely, professional, and compassionate manner.

Build and maintain strong, trust-based relationships with patients and caregivers.

Consistently deliver a high-quality patient experience aligned with program standards.
Qualifications

Bachelorโs degree in health sciences or related field preferred.

NBC-HWC (National Board for Health & Wellness Coaching) certification required.

Experience in a healthcare, chronic disease management, or patient engagement setting preferred.

Cardiology or heart failure experience a plus.

Telehealth or remote patient engagement experience preferred.

Strong communication and interpersonal skills.

Ability to motivate and support behavior change in diverse patient populations.

Organized, adaptable, and comfortable managing multiple priorities.

Proficiency with electronic health records and standard computer applications.

Bilingual English/Spanish proficiency preferred.
