Role Description

As a Health Coach at Vori Health, you will play a vital role in empowering our members to take ownership of their health. You will work closely with a trained care team to implement modern behavioral change strategies and support patients in achieving their wellness goals.

• Consistent use of Vori Health’s behavior change solutions.
• Implement coaching techniques to improve patient outcomes.
• Provide video consultations and messaging with patients to improve their wellness goals.
• Design and support patient care plans utilizing motivational interviewing, SMART goals, and behavior change techniques.
• Manage a substantial patient caseload.
• Document interactions and interventions as directed by Vori Health technology tools.
• Collaborate effectively with cross-functional teams.
• Engage regularly with physicians, physical therapists, and other providers.
• Troubleshoot unexpected issues to prioritize patient needs.
• Utilize critical thinking to discern appropriate referrals among clinical team members.
• Assist in the development of technology and service innovations.
• Provide feedback on individual and team performance.
• Communicate and collaborate with external clinicians as appropriate.
• Maintain professional knowledge through educational workshops and publications.
• Meet company and quality standards as defined by Vori Health.
• Perform other projects and duties as assigned.

Qualifications

• Board Certified Health and Wellness Coach.
• Proficiency with computers, virtual care, database management, and data tracking.
• Knowledge of Motivational Interviewing and establishing SMART goals.
• 2+ years of experience as a health coach or wellness educator.
• Demonstrated competence in digital care delivery or telehealth.
• Outstanding written and spoken communication skills.
• Proven record of matching patient needs to treatment plans.
• Strong sense of empathy and ability to motivate patients.
• Proficient with Microsoft Office Suite or related software.
• Reliable high-speed internet connection.
• Dedicated home office space, private and free from distractions.
• Comfortable working in a fast-paced startup environment.
• Excellent time management skills.
• Strong communication and collaborative working style.
• Proven record of creating exceptional patient experiences.

Requirements

• Bi-lingual, Spanish speaking (nice-to-have).
• Experience in working toward specific outcomes in patient achievement.
• Understanding of behavioral interventions and approaches to behavior change.
• Skilled in using Canva to create actionable tools and guides.
• University degree in health science or social science discipline.
• Ability to troubleshoot unexpected issues.
• Willingness to provide support to patients and clinical care team.
• Ability to work in multiple technology platforms simultaneously.
• Attention to detail while seeing the big picture.
• Excellent interpersonal, organizational, and problem-solving skills.
• Ability to prioritize tasks and delegate when appropriate.
• Critical thinking skills and flexibility in a fast-paced environment.
• Experience working closely with clinical teams.
