[Hiring] Eligibility Specialist @Form Health

Role Description

We are looking for an Eligibility Specialist who will be responsible for supporting patients with determining program eligibility, collecting all necessary records to confirm eligibility, and keeping patients informed throughout the entire process. This role will help us ensure patients get started with the program and their care in a safe and streamlined way. This will be a full-time opportunity, reporting to Vanessa Ramirez, Manager, Eligibility. This is a remote position located anywhere in the USA.

• Eligibility Review: Detailed review of patient records, clinical documentation, and administrative data to accurately determine eligibility for Form.
• Record Collection: Manage all work related to requesting, faxing, and tracking medical records from external Primary Care Providers (PCPs) and specialists, ensuring HIPAA and other confidentiality/privacy compliance during the record collection process.
• Communication: Maintain professional and clear communication with patients and colleagues, keeping patients up to date throughout the eligibility process.

Qualifications

• Demonstrated experience in a role requiring detailed medical record review and collection.
• Proficiency with electronic medical record (EMR) systems.
• Exceptional organizational skills and attention to detail.
• Strong communication skills, both written and verbal, for corresponding with patients, clinicians, external providers, and colleagues.
• Ability to manage and track multiple tasks simultaneously.
• Ability to work independently and collaboratively.
• Working knowledge of HIPAA regulations.
• Active Medical Assistant Certification.
• At least 1 year experience in a primary care, endocrinology, or urgent care setting.
• At least 1 year in a remote care setting or work from home environment (preferred).
