Role Description

Under the general supervision of the Director and Clinical Manager, responsible for providing quality patient care, virtually, using the nursing process and comprehensive care coordination in patient populations as assigned in accordance with established policies, procedures, regulatory and accreditation requirements as well as applicable professional standards including telehealth nursing guidelines. Coordination and outcome measurements are provided via audio/visual technology, collected, and other interfaced data elements in a remote operations center. Employee is subject to call back and overtime as required by the hospital.

• Participates in population health management across the continuum to achieve optimal clinical, financial, operational, and patient satisfaction outcomes.
• Collects and reviews patient information while applying appropriate standards of care, including:
  • Reviewing patient history and physical.
  • Assuring admission data entry for the Telehealth database is completed within the appropriate timeframe.
  • Completing audio visual assessments and reassessments according to patient need and Telehealth standards of care.
• Evaluates and collaborates with the interdisciplinary team in a timely manner while monitoring patient progress towards goals, responses to treatments/interventions, and expected outcomes.
• Reviews and suggests revisions to the plan of care to reflect patient needs based on evaluation of the patient’s ongoing condition(s).
• Documentation reflects objective/subjective data and, if applicable, patient’s response, and is timely in accordance with applicable policy(s).
• May participate in research activities and unit or facility shared leadership.
• Interacts with patients, families, and visitors regarding technology and the role of Telehealth.
• Serves as a resource to other staff members and facility-based clinical teams while precepting and mentoring when appropriate.
• Uses skilled communication and promotes collegiality between staff, providers, and leaders to achieve optimal clinical and operational outcomes.
• Provides excellent service experience to all customers of West Tennessee Healthcare by consistently demonstrating WTH core and leader behaviors.
• Maintains professional and technical knowledge by attending educational workshops and completing all mandatory education.
• Interacts with all levels of staff in various departments and facilities.
• Communicates with patients and their families, collaborating with other disciplines for emotional support, teaching, and discharge planning.
• Assists in orientation of new personnel to the unit and serves as a role model as required.
• Communicates information regarding patients and unit operation to other shifts and managers, using proper channels of communication to other departments of the hospital.
• Performs related responsibilities as required or directed.

Qualifications

• Completion of a diploma, associate, or baccalaureate degree in nursing required.
• Current licensure as a Registered Nurse as approved by the Tennessee Board of Nursing for practice in the state of Tennessee.
• Responsible for Continuing Education Units annually.
• BLS certification must be obtained within the orientation period and kept current while in this position.
• In Critical Care areas, ACLS certification must be obtained within the first year of employment and kept current; in other areas, ACLS is preferred.
• Certification in area of clinical expertise by recognized authoritative agency desirable.
• Additional certifications may be required at department’s discretion.

Requirements

• 2 years of critical care experience required; 5 or more years of experience preferred.
• Skill and proficiency in the principles, concepts, and techniques of the nursing profession as normally acquired through the completion of a diploma, associate, or baccalaureate degree in nursing.
• Knowledge of policies and procedures of the hospital and completion of skills check sheet for designated areas as acquired through a 3-month probationary period.
• Must be able to demonstrate EKG proficiency; additional certification or continuing education may be required based on area of practice.
• Must demonstrate critical thinking, effective communication skills, problem-solving abilities, customer service, collaboration, time management skills, and telehealth computer program literacy.
• Basic computer skills preferred.
