Role Description

Join a fast-growing healthcare organization improving care gap performance across telehealth and in-home care.

• Own care gap quality, documentation integrity, and operational performance.
• Work directly with Clinical Operations, Compliance, Analytics, and Provider Leadership.
• Help scale compliant, defensible care gap programs across distributed clinical teams.

What You'll Do

• Lead care gap closure and integrity initiatives
• Improve documentation quality and workflow consistency
• Monitor provider performance and operational trends
• Build quality assurance and validation processes
• Support audit readiness and compliance activities
• Coach providers on documentation and HEDIS standards
• Partner cross-functionally to improve care gap completion

Qualifications

• Active NP license required
• 5+ years clinical operations or quality leadership
• Medicare Advantage or Medicaid program experience
• Strong HEDIS/STARS knowledge
• Experience leading provider performance initiatives

Nice to Have

• In-home clinical program experience
• Telehealth operations experience
• Audit response or QA program leadership
• Healthcare analytics/reporting tools experience
