Role Description

Serves as a designated expert consultant for the Clinical Quality function, contributing to strategic direction to facilitate the achievement of company goals and clinical priorities for improved patient outcomes and superior quality standards for assigned group(s).

  • Provides expertise, support, and guidance to assigned operations teams and clinical quality teams regarding continuous Quality Assessment and Performance Improvement (QAPI).
  • Ensures effective communication among the interdisciplinary team (IDT) and produces quality assessment and performance improvement activities that positively influence clinical quality outcomes.
  • Covers home hemodialysis and home peritoneal dialysis, responsible for monitoring data/information, prioritizing areas for improvement, and developing plans to improve clinical quality outcomes.
  • Provides strategic leadership of short- and long-term goals to implement positive change and create organizational structure.
  • Drives high-quality performance through development and implementation of strategies related to quality outcomes.
  • Collaborates with stakeholders including Clinical Quality leadership, Operations, Education, Clinical Services, and others to facilitate achievement of quality goals.
  • Responsible for training and mentoring direct reports in Quality Improvement concepts.
  • Oversight for execution of Quality and clinical initiatives with clinic teams.
  • Mentors Clinical Quality Team to collect, trend, and analyze data to monitor effectiveness of clinical and operational processes.
  • Contributes to the development of goals related to quality outcomes, patient safety, and patient-centered care.
  • Incorporates QSEN competencies into practice to continuously improve quality and safety of care.
  • Technically proficient in utilizing dashboards to monitor quality trends and analyze complex quality issues.
  • Participates in the development and execution of quality improvement programs and initiatives.
  • Collaborates with Clinical Quality leadership to design training programs to reduce risk exposure.
  • Responsible for national delivery and maintenance of assigned training programs for clinical staff.
  • Maintains continuing education documentation for training programs to ensure compliance.
  • Utilizes data surveillance to analyze quality outcomes and provide updates to stakeholders.
  • Interacts frequently with internal departments for problem resolution.
  • Responsible for hiring, coaching, and counseling employees.
  • Collaborates with Medical Directors and Regional Operations to achieve positive patient treatment outcomes.
  • Utilizes evidence-based practices to address barriers to quality improvement.
  • Attends and participates in team meetings for quality team building and staff development.
  • Provides guidance and subject matter expertise to operations teams regarding quality-related policies and procedures.
  • Facilitates implementation of company-wide policies and procedures into standard operating procedures.
  • Utilizes quality improvement principles to evaluate impact of strategies and initiatives.
  • Applies knowledge of clinical and administrative policies, CMS Conditions of Coverage, and quality improvement concepts.
  • Collaborates with VBC team to plan process improvement interventions.
  • Utilizes adult education principles in the development and execution of education programs.
  • Accountable for outstanding customer service to all customers.
  • Develops and maintains exceptional relationships through effective communication.
  • Performs other related duties as assigned.

Qualifications

  • Registered Nurse required.
  • BSN or bachelor’s degree in healthcare-related field preferred or an equivalent combination of education and experience.
  • Certification in Nephrology Nursing or quality preferred.

Requirements

  • 6+ years of Nephrology Nursing and/or related nursing experience.
  • 3+ years' experience in a leadership role.
  • Strong interpersonal skills with the ability to communicate with all levels of management.
  • Excellent oral and written communication skills.
  • Strong management skills with the ability to lead cohesive and productive teams.
  • Strong organizational, critical thinking, and customer service skills.
  • Demonstrated adaptability to changes in priorities.
