Role Description
As a Care Navigator, you will be an invaluable member of our team, ready and eager to make an impact internally and to patients we serve. You will support patient success by:

Identifying the needs and challenges patients have and assisting in implementing interventions to guide new thinking and create behavior change.

Providing remote monitoring services to help support patients as they progress through their home-exercise therapy program.

Directing and assisting users in therapeutic exercise, neuromuscular re-education, and functional training through our technology platform.

Providing ongoing feedback to the evaluating clinician on patient progress outside of the clinic.

Maintaining necessary records and notes while providing remote monitoring.

Assisting with improving processes and functionality of the platform.

Organizing and managing a caseload while staying efficient.
Qualifications
-
2+ years of experience in musculoskeletal health is a plus.

Graduated from an accredited physical therapy program.

Active PT license in a compact state (https://www.ptcompact.org/).

Tech Savvy - thrive in a multi-platform environment, comfortably switching between systems (EHR, messaging, scheduling) without missing a beat.

Experience in behavioral modification; Health Coach Certification is a plus.

Excellent communication skills, interpersonal, and presentation skills, and an ability to collect and present meaningful data to drive decisions.

Strong relationship-building and listening skills.

Demonstrated patient focus; a genuine passion for the user and their experience.

Ability to multi-task and manage various types of users.

Ability to work independently with limited supervision.

The flexibility and agility to work cross-functionally in a fast-paced environment.
