Role Description

We are delighted to offer an excellent opportunity for a NMC Registered Band 5 Triage Nurse to join our friendly and supportive East Staffordshire Care Coordination Centre in Burton. We are looking for a passionate nurse who is innovative, motivated and enthusiastic with excellent communication and organisational skills to contribute to the delivery of a high-quality service within the community nursing environment.

• Contribute to the effectiveness and efficiencies of community nursing teams by supporting triage of calls and referrals.
• Actively liaise with patients, carers, and service providers to support clinical decision making.
• Make appropriate decisions following comprehensive remote triage using available information from referral information and electronic clinical systems.
• Encouraged to develop skills via CPD programmes, with support from peers and team leaders.

Qualifications

• Professional and clinical knowledge of community nursing.

Requirements

• Understand the scope of the community nursing service and the role of other MPFT and external services within each Primary Care Network.
• Occasional delivery of care to patients in a community setting to maintain up-to-date clinical experience.
• Maintain up-to-date knowledge of clinical aspects of community nursing to resolve clinical queries over the phone.
• Supervisory responsibilities in the absence of the triage lead nurse, including coordinating the team and allocating work.
• Occasional supervision of students and new nurses joining the community nursing team.
