Role Description

American Senior Communities is now hiring a Community Nurse Liaison (LPN).

Hours: Saturday and Sunday 8a - 5p

Remote position but must live within reasonable driving distance to Indianapolis.

• The Clinical Nurse Liaison provides patient evaluations.
• Collaborates with hospital personnel to determine patient’s clinical needs and appropriateness for admission to skilled nursing facility.
• Works closely with our communities and healthcare partners.
• Checks benefits and payor sources.

Qualifications

• Minimum of three years of clinical experience in acute care or long-term care setting.
• Current Indiana LPN License.
