Role Description

This position is Remote, however; the Candidate will be expected to reside in and travel throughout Pierce County.

The Community Health Worker serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as:

• Outreach
• Community education
• Informal counseling
• Social support
• Advocacy

To be successful in this role, you:

• Have three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness.
• Are comfortable working with at-risk populations, communities of color and sourcing local resources for persons with behavioral and/or physical health needs.
• Have experience using a health plan care management system or electronic medical record system.
• Have a high school diploma or equivalent.
• Are a certified Peer Specialist if you do not have minimum experience and education requirements.
• May have other special skill sets such as speaking another language or certification as a Community Health Worker.
• Have a valid state issued driver’s license and maintain an acceptable driving record and transportation.

Responsibilities

• Engage prospective care management program participants identified through data analysis and referral sources.
• Provide community outreach services including home visits, assisting individuals with accessing transportation services, educating enrollees on healthy behaviors, and providing information on community resources.
• Provide follow-up services via telephonic or face-to-face engagement with clients and service planning partners.
• Collaborate with care managers in securing and identifying needed referrals to community and network medical, behavioral health and social assistance providers.
• Provide oral and/or written status updates regarding client alerts, progress and needs to responsible care managers and providers.
• Ensure case documentation is consistent with policies and procedures.
• Provide scheduled activities that promote socialization, recovery, self-advocacy, development of natural supports, and maintenance of community living skills.
• Provide information to increase the enrollee's knowledge about health conditions and improve adherence to prescribed treatment.
• Travel on behalf of the Company and work in the field at least 50% of the time.
• Other duties as assigned.

Requirements

• Complete and successfully pass a criminal background check.

Compensation and Benefits

• The position is FLSA Non-Exempt and is eligible for overtime with a 5% annual incentive target based on performance goals.
• Base pay will take into account internal equity and may vary depending on the candidate’s job-related knowledge, skills, and experience.

• Sensory: Speaking, hearing, near vision, far vision, depth perception, peripheral vision, touch, smell, and balance.
• Physical: Extended periods of sitting, computer use, talking, and possibly standing; simple grasp, firm grasp, fine manipulation, pinch, finger dexterity, supination/pronation, wrist flexion.
• Mental: Ability to learn and prioritize multiple tasks and handle demanding situations; analytical/problem solving/critical thinking ability.

Work Environment

Office environment. Employees who frequently work in front of computer monitors are at risk for environmental exposure to low-grade radiation.
