[Hiring] Community Health Program Coordinator @IMPaCT Care

Role Description

As a CHW Program Coordinator, you are the operational engine behind our CHW program in Georgia. You connect eligible patients to life-changing care, keep data clean and actionable, and ensure the program runs smoothly — even when things get complicated. You work closely with CHWs, Field Managers, and clinical partners, bridging the gap between people and the systems that support them.

Qualifications

• 1–3 years of experience in program coordination, research administration, or equivalent
• Demonstrated experience with data collection, analysis, and protocol implementation
• Proficiency in Microsoft Excel (reporting, data organization, pivot tables)
• Strong written and verbal communication skills
• Excellent organizational skills and keen attention to detail
• Ability to work independently and solve problems effectively
• Flexibility and adaptability in a fast-paced environment
• Ability to work on-site in Atlanta, GA

Requirements

• Bachelor's Degree in Public Health, Social Work, Healthcare Administration, or a related field (preferred)
• Experience with an EMR system such as EPIC or case management platforms (preferred)
• Familiarity with SDOH screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) (preferred)
• Experience with program or research enrollment workflows (preferred)
• Knowledge of Medicaid, D-SNP, or public benefits programs (preferred)
• Comfort working in community health, social services, or healthcare settings (preferred)
