Role Description
Clinical Program Specialist β Population Health supports the execution of population health initiatives across the West Region by translating data into actionable insights that improve clinical outcomes and care delivery. This role partners with clinical, operational, and analytics teams to identify performance gaps, support care gap closure strategies, and enable effective implementation of population health programs.

Primary Responsibilities:

Analyze population health data to identify care gaps, utilization trends, and performance opportunities

Support execution of programs focused on chronic disease management, preventive care, and care coordination

Translate data findings into clear, actionable recommendations for clinical and operational stakeholders

Partner with provider groups and market teams to support quality improvement efforts (HEDIS/STARS), medical management, and accurate clinical documentation

Develop and maintain reports, dashboards, and performance tracking tools

Monitor program performance and support continuous improvement efforts

Identify opportunities to enhance member engagement and clinical outcomes

Support implementation of best practices and standardized workflows across markets

Communicate findings and recommendations to stakeholders in a clear and structured manner
Qualifications

3+ years of experience in healthcare analytics, population health, or healthcare operations
-
Experience working with healthcare data (claims, EMR, population health platforms, care management programs, or other clinical reporting)

Experience partnering with provider groups and market teams to support medical management, clinical affordability, or other value-based programs

Demonstrated experience analyzing complex data and identifying root causes and recommending solutions that improve process efficiency, quality, or business outcomes

Experience working independently and manage multiple priorities

Experience translating data into insights and effectively communicating to stakeholders and leadership

Advanced Excel, data visualization/analytics (Tableau, PowerBI), Smartsheets, and PowerPoint skills
Requirements

Knowledge of evidence-based clinical guidelines used by medical groups, health plans, hospitals, and utilization management teams
