[Hiring] Clinical Intake Coordinator @Trinity Health

Role Description

RN Clinical Intake Coordinator needed for Mercy Home Health.

• Responsible for the efficient and effective facilitation of referrals to the post-acute homecare/hospice agency once identified in the acute care setting.
• Provision of education to acute care staff, including physicians, regarding services available from homecare/hospice.
• May also be involved in business development activities as they relate to business expansion or new program development.

Qualifications

• Graduate of an approved clinical education program.
• Current clinical RN license in the state of Pennsylvania.
• Current clinical LPN License in the state of Pennsylvania.
• Minimum of two (2) years' experience of professional clinical experience in an acute care setting.
• Minimum of two (2) years' experience of professional clinical experience in a home care setting, or 2 years’ experience in a sales or marketing role.
• Must have current Driver’s license and reliable transportation to and from work site.
• Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.

Requirements

• Ability to work an Afternoon shift from 11:30am-8:00 PM Monday-Friday.
• Job is currently remote but must be within driving distance to a Trinity Location in Philadelphia.
