Role Description
The Clinical Dietician works with patients and the clinical team to provide personalized nutrition recommendations to promote patient healing. This includes educating patients and families with practical dietary suggestions, being mindful of patient capacity for growth in the context of their cultural and emotional environment.

Requirements

Nutrition Consults are addressed within the day following consult request and consult completion is documented in the electronic medical record (EMR).

Nutrition Assessments for SJRMC and SJRRH patients identified at nutrition risk are completed within two days and entered into the EMR.

The plan of care for patients identified as at nutrition risk is re-evaluated and documented within at least 1 week of the initial assessment.

Provide nutrition education to inpatients and outpatients to facilitate self-management of medical nutrition therapy.

Design and present nutrition classes (e.g., cardiac, diabetes, pulmonary, weight management) and document education in the EMR within 24 hours of service.

Provide education to outpatients and document individual medical nutrition therapy to provider office within one week.

Develop and present timely nutrition topics based on community need.

Attend committee meetings as an active participant and serve as an education resource to other staff.

Participate in projects involving clinical and information technology overlap; assist with staff training as assigned.

Respond in a timely manner to phone inquiries, attempting to return calls within 24 hours of receipt.

Enhance professional growth and development to maintain current working knowledge, CDR registration, and state licensure.

Attend department in-services and meetings; participate in QA/PI programs when assigned.

Complete all annual HealthStream modules 100%.

Implement SJRMCβs Service Standards into daily work: Safety, Courtesy, Effectiveness, and Stewardship.

Other duties as assigned.
