Role Description

This position is for 1 part time 30 hours per week post, some weekend working is required. This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.

• UCR/HaH is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis.
• The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.
• This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.

Requirements

• Manage the case load of clinical staff within the Urgent Community Response (UCR) and Hospital at Home (HaH) Team.
• Gather, input and provide essential data for patients, helping to maintain accurate records.
• Provide administration support and caseload co-ordination within the team.
• Answer telephone calls from patients and pass information onto a clinician.
• Ensure that the team case load is kept up to date.
• Participate and co-operate with the Multi Disciplinary Team to contribute to the effectiveness of the service.
• Use multiple patient information management systems (EMIS, Docobo, Patient Centre etc.) to enter required information.
• Maintain an accurate database of home monitoring devices.
• Participate in and organise team audits when appropriate.
