Role Description

As a Telephonic RN Case Manager, you’ll support a diverse member population with education, advocacy, and connections to the resources they need to feel better and get well by outreaching post discharge or with a disease-specific focus. This is a high volume, member-focused environment where you’ll need to be efficient, productive, and thorough while working with our members over the phone.

Hours: Monday - Friday: 8:00 am to 5 pm in local time zone and may require one evening per week.

You’ll enjoy the flexibility to work remotely from anywhere within the U.S. as you take on some tough challenges.

• Provide a complete continuum of quality care through close communication with members via manual outbound calls and progressive dialer calls.
• Manage and coordinate care and services with interdisciplinary care team to identify and close gaps or barriers in treatment plans.
• Engage and collaborate with patient and/or caregiver to develop individualized Care Plan.
• Support members with health education, medication reviews, and referrals to community resources.
• Act as an advocate for the patient.
• Maintain HIPAA compliance at all times.

Qualifications

• Current, unrestricted RN license in a compact state of residence with the ability to obtain multiple state licenses as required.
• 3+ years of experience as a Registered Nurse in a hospital, acute care or direct care setting.
• Intermediate/Proficient computer skills: internet navigation, talk and type at the same time, Microsoft Office (Outlook, Word, Excel), navigate a Windows-based environment.
• Ability to handle multiple tasks and function in a high-volume environment.
• May be required to obtain a Certified Case Management certification within 30 months of employment.
• Dedicated, distraction-free workspace in home for home office.
• Access to high-speed internet.

Requirements

• Certified Case Manager (CCM) (preferred).
• Experience in case management (preferred).
• Proven ability to educate patients on health and wellness (preferred).
• Proven organizational skill and ability to complete all work independently and within designated timeframes (preferred).
• Demonstrates being adaptable, flexible, and able to maintain a positive attitude during changes in process, practice or policy (preferred).
• Spanish speaking (preferred).
