[Hiring] Case Manager - Registered Nurse @CVS Health

Role Description

We’re looking for a compassionate and detail‑driven Registered Nurse (RN) to join our team as a Case Manager RN. In this role, you’ll help guide patients through their healthcare journeys, ensuring seamless transitions, advocating for their needs, and improving outcomes through thoughtful care planning and collaboration.

• Coordinate and manage patient care plans across healthcare settings
• Assess patient needs using clinical data, history, and diagnostics
• Collaborate with providers and care teams to ensure effective communication
• Support safe, timely transitions from hospital to home
• Conduct utilization reviews to promote high-quality, cost‑effective care
• Track patient progress and manage disease and case management workflows
• Leverage digital tools and analytics to monitor performance and outcomes
• Connect patients with community resources to enhance care

Working scheduled is 8:00 am to 4:30 pm in your time zone.

Qualifications

• Active & unencumbered RN license (required) in the state of residence
• 3+ years of acute care clinical experience as an RN
• Strong problem-solving and decision-making skills
• Knowledge of medical terminology and clinical workflows
• Comfort with digital tools and healthcare systems
• Ability to communicate clearly and handle sensitive information with care
• Passion for patient advocacy and working with diverse populations

Requirements

• A compact RN license is preferred
• Managed care experience preferred
• Home Care experience preferred
• Certified Case Manager (CCM) preferred
• Associates Degree in Nursing required, BSN preferred
