Role Description
The Virtual Intensive Outpatient Program (VIOP) Case Manager/Group Facilitator provides ongoing care coordination, advocacy, resource support, engagement support, and recovery-oriented services for individuals participating in virtual substance use treatment services.

Assist individuals in reducing barriers to treatment engagement, recovery stability, emotional wellness, and independent functioning.

Coordinate care between internal providers, legal systems, social service agencies, recovery supports, and community resources.

Support client retention, attendance, discharge planning, relapse prevention, and overall recovery stabilization within a telehealth treatment environment.

Assist clients in navigating telehealth platforms and reducing technology-related barriers that may interfere with treatment participation and continuity of care.

Support virtual group operations by assisting with attendance monitoring, engagement support, client communication, crisis escalation concerns, and coordination with clinical staff during treatment services.
Core Responsibilities

Conduct assessments to determine the needs of clients seeking behavioral health and addiction recovery services.

Develop individualized treatment plans in collaboration with clients, healthcare providers, and other stakeholders.

Coordinate care and services provided to clients, including therapy and group therapy.

Monitor and evaluate clients' progress towards treatment goals and make adjustments to the treatment plan as needed.

Advocate clients to ensure they receive appropriate care and support from healthcare providers and community resources.

Provide education and support to clients and their families about behavioral health issues, recovery addiction, and coping strategies.

Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress reports.

Collaborate with a multidisciplinary team of healthcare professionals, including clinical and medical teams.

Assist clients in identifying and reducing barriers impacting treatment participation and recovery stability.

Assist with observed oral toxicology swab collections in accordance with agency policy, confidentiality standards, and chain-of-custody procedures.

Stay informed about current trends and best practices in behavioral health, addiction recovery service, telehealth services, and virtual treatment operations.

Attend and actively participate in required training and staff meetings.

Flexibility to adapt to schedule changes and assume responsibilities not delineated in the job description.
Qualifications

Required: HS Diploma or GED
-
Preferred: Bachelor's Degree or higher in a Human Services field.
License Requirements
-
CDCA license valid in the state of Ohio is required.
Experience Requirements
-
Preferred 3+ years of experience in addiction recovery or behavioral health settings.

1 year of Case Management experience preferred.

Excellent communication and interpersonal skills.

Knowledge of evidence-based practices in behavioral health and substance use treatment.
