Role Description
In this role you provide administrative and case management support to a team of 4-5 Behavioral Health Case Managers who oversee family clinic sites of Children's Health.
Provides case management services to an assigned patient population, including but not limited to:

Provides support that requires clinical expertise in various systems with focus on helping the family negotiate the complexities involved with a diagnosis. Each BHCC provides case management support to patients and is responsible for:

Providing social service referrals for patients and families, including housing, transportation, medical and mental health services

Ensuring the preparation and maintenance of required documentation per accreditation standards and CHST policy

Working closely with a team of Behavioral Health Care Managers and consulting psychiatrists to ensure patients adhere to their plans of care

Communicating with patient families, identifying barriers to care, and helping to ameliorate those barriers when possible

Being part of a multi-disciplinary treatment team and building collaborative relationships with UTSW providers
Qualifications

At least 3 years previous experience in social services or healthcare or related experience required

Four-year Bachelor's degree or equivalent experience required

Graduate or professional work or advanced degree; or equivalent experience in Social Work, Psychology, Marriage and Family Therapy or related discipline preferred
Requirements

Attends and participates in team meetings with team lead as well as monthly departmental discussions
-
Collaborate with inpatient treatment team to prevent gaps in care and provide discharge planning support

Assists patient families navigate the behavioral health delivery system based on their clinical need and financial/insurance coverage

Facilitate parent/child referrals to address social service needs and supportive therapy

Coordinate and schedule psychiatric consultations, ongoing care with licensed providers, LMHAs, and support groups

Support licensed behavioral health clinicians by outreaching families under their supervision

Assists in identifying patientβs insurance coverage and facilitate access to health information, community resources, and social services

Participate in rounds/treatment/consultation meetings as well as monitor the patient's progress toward the desired outcome

Collaborate with providers to establish treatment milestones to prepare patient and family for transition to lower level of care

Oversee the flow of treatment information across providers and within the care team in order to maximize patients' physical and emotional well-being

Facilitate referrals to the appropriate areas to expedite care, treatment and services

Participate in the development, implementation, evaluation and ongoing revision of the plan of care, clinical pathways and initiatives to improve quality, continuity and cost effectiveness

Assesses patient functioning and escalates to the treating Behavioral Health Care Manager or consulting psychiatrist when clinically appropriate

Work collaboratively with other departments and services to define and study areas of inefficiency and participate in process improvement projects

Be involved in the development of strategies and plans to maximize the most appropriate use of services in assigned areas

Submit to and pass external background check processes as required per vendor or funder requirements
