[Hiring] Case Manager @Embark Behavioral Health

Role Description

Embark Behavioral Health is growing and seeking a Virtual Case Manager to join our telehealth-based IOP/PHP program serving teens, young adults, and their families. This role supports clients both clinically and administratively while partnering closely with therapists, leadership, and families to ensure coordinated, high-quality care. Our Virtual IOP program plays a critical role in expanding access to life-saving mental health treatment.

• Coordinate client schedules, attendance, and clinical documentation
• Complete education assessments, Education Success Plans, and school communication
• Serve as primary liaison for families, schools, and clinical team
• Support admissions, pre-treatment, case consults, and discharge planning
• Assist with PHP/IOP scheduling, group facilitation, and case management
• Manage consents, benefits verification, outcome measures, and EHR documentation
• Coordinate medication management and access to health services
• Participate in staff meetings, supervision, and assigned duties

Qualifications

• Bachelor’s degree in a social services–related field or equivalent experience (Master’s preferred)
• Experience in behavioral health and/or education strongly preferred
• Strong organizational, communication, and time-management skills
• Ability to remain calm under pressure, maintain boundaries, and resolve conflict
• CPR/First Aid certification (or willingness to obtain) and ability to pass a background check
