[Hiring] Case Management Coordinator II (RN) @Health Care Service Corporation

Role Description

This position is responsible for performing telephonic outreach for recently discharged and high-risk patients. Key responsibilities include:

• Facilitating discharge coordination and identifying alternate treatment programs that provide medically necessary potentially cost-effective services.
• Consulting with physicians, providers, members, and other resources to assess, plan, facilitate implementation, coordinate, monitor, and evaluate options and services required to meet an individual’s health needs.
• Using communication and available resources to promote quality, cost-effective outcomes.
• Serving as a liaison to physicians and members.
• Providing telephonic support for all locations.
• Ability and willingness to travel within assigned location.

This position may be performed remotely from anywhere within the continental United States, excluding California, New York, Alaska, and Hawaii.

Qualifications

• Strong communication skills.
• Experience in healthcare or related field preferred.
• Ability to work independently and as part of a team.
