Role Description

Work remote on a consistent PST schedule with a capped caseload—focus on patients, not paperwork.

Role: Care Navigator

Location/Type: U.S. • Remote (PST hours)

Pay: $46,000 salary (CA: ~$25/hr)

Schedule: 9am–6pm PST

What you'll do:

• Make 20–70 outbound calls/day; land ~10 patient check-ins
• Schedule visits, labs, imaging, and outside referrals
• Coordinate care between providers and patients (clear ownership)
• Gather medical history and document in EHR
• Follow up after visits with next steps
• Escalate issues to clinical team (fast feedback)
• Keep patients engaged and supported on calls

Qualifications

• Comfortable making high-volume outbound calls daily
• Basic medical terminology knowledge
• Experience speaking with patients/customers by phone
• Clear, professional communication skills
• Able to adapt in a fast-changing environment

Requirements

• Care coordination or patient navigator experience (nice to have)
• Spanish language skills (nice to have)
• Experience with cognitive/elder care populations (nice to have)
