Role Description

Sentara Health Plans is hiring a Care Coordinator. The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Responsibilities include:

• Developing, participating, and monitoring multidisciplinary collaboration of services to patients where appropriate.
• Assisting adult patients and their families with personal and environmental difficulties associated with medical conditions, including terminal illnesses.

Qualifications

• Must possess a degree in Health & Human Services or one of the following related fields:

• All degrees must be from accredited schools listed on the U.S. Department of Education College Accreditation database.
• Degrees from schools outside the United States will be reviewed individually.

Requirements

• Long Term Care - 1 year (required)
• 3 years of experience in care coordination (preferred)
• Ambulatory setting experience (preferred)
• Health Plan - 1 year; Medicaid - 1 year (preferred)
• Certified Health Coach or Certified Medical Assistant (preferred)
