Role Description

Sentara Norfolk General Hospital has a full-time APP to join Sentara's Case Management Pharmacy Program, VA. Work from home or at the hospital! Remote work available.

• Join an experienced team of 1 MD, 9 nurse case managers, 5 clinical pharmacists, and 1 nurse supervisor
• Primary Location: Sentara Norfolk General Hospital
• Schedule: Mon – Fri, 8:30AM – 5:00PM, no weekends or holidays and working from home is available
• Duties:
  • Perform health risk assessments, re-write prescriptions
  • Review 150 patients per month
  • Work with Sentara Medicaid only
• 1 year experience required as APP in Family Medicine
• Experience in Rheumatology or Infectious Disease preferred
