Role Description
The Veteran Care Navigator (VCN) is responsible for in-person veteran peer support, assisting veterans and members of the armed services, facilitating access and transitions in care.
- Connects with veteran organizations, educating the community on services available through CCBHC and facilitating referrals into care.
- Supports the development of the Certified Community Behavioral Health Clinic (CCBHC) to enhance and advance CCBHC requirements.
- Ensures clients are receiving care as necessary to meet CCBHC core service obligations and in compliance with standards, regulations, and laws.
- Works collaboratively with CCBHC clinical teams, ensuring individuals served are engaged with treatment and other resources.
- Consults with CCBHC leadership and community resources to ensure services are engaging and impactful for clients.
- Cultivates a safe and supportive network in conjunction with provision of recovery education and advocacy using trauma-informed, person-centered, and shared-decision making models.
Qualifications
- Bachelor's Degree in human services field or 5 years of experience in the behavioral health field
- Military Veteran status required
- State Certified Veteran Support Specialist, or willing and able to obtain the certification upon hire
Requirements
- Military cultural competency and a well-developed understanding of resources within and outside the CCBHC
- Engaging and energetic demeanor with skills in building professional community relationships
- Skills in customer service techniques and professional etiquette
- Skill in responsible team collaboration, time management, and prioritizing tasks
- Ability to problem solve complex issues and deliver effective decision making
- Knowledge of community resources specifically related to Veteran Serving Organizations
- Ability to effectively communicate via written, verbal, in-person, and virtual methods
- Ability to attend in person meetings
