[Hiring] Intake Coordinator @Modivcare

Role Description

Modivcare Personal Care Service is looking for an Intake Coordinator. In this role, you will help ensure new client referrals are processed quickly and accurately while supporting a positive experience for clients, caregivers, and referral partners.

• Answer and manage incoming referral leads through phone, email, and payor portals
• Gather patient information, verify payer requirements, and obtain initial authorizations
• Maintain accurate patient, authorization, and scheduling records in internal systems
• Coordinate with Care Coordinators and branch teams to ensure smooth client and caregiver onboarding
• Build and maintain strong relationships with referral sources and respond promptly to requests
• Ensure compliance with HIPAA, company policies, and applicable regulations
• Support additional operational duties and projects as assigned

Qualifications

• High school diploma or equivalent
• 1+ years of direct intake experience
• 1+ years of home care experience
• Knowledge of Medicaid eligibility and payer sources required

Requirements

• Pay: $24.68–$26.50, depending on experience
• Schedule: M-F 8:00AM-5:00PM EST
• Office Address: Remote in MA
