Role Description

If you are passionate about helping others break free from the grip of addiction and guide them on their journey to recovery, we invite you to bring your expertise and heart to our team as a Substance Use Counselor. In this rewarding role, you’ll have the opportunity to work closely with individuals struggling with substance use on an outpatient basis, offering them the support and tools they need to rebuild their lives with a more hopeful perspective.

Provide individual counseling for adult clients seeking treatment for substance use disorders and/or medication assisted treatment on an outpatient basis.
Complete intake assessments for individuals seeking SUDS/MAT services to determine appropriate level of care and requirements of program are met prior to admission.
Provide therapeutic interventions to promote personal growth and resiliency in clients with a primary behavioral health diagnosis.
Perform continuing assessment of the client’s needs and functioning, maintenance of appropriate documentation, and development and implementation of personal growth plans consistent with the client’s therapeutic needs.

Essential Duties & Responsibilities

Facilitate therapy sessions for a client or group of clients.
Assist the care team in the development of treatment plans and modify plans over time in response to client progress.
Conduct intake for recipients entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned.
Conduct assessment and write assessment summaries, as assigned.
Assist clients with making linkages with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.
Maintain appropriate contact with referral sources including providing information on client progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
Assist clients in making discharge plans and provide follow-up services as assigned.
Provide ongoing clinical supervision of practicum and internship students, and staff, as qualified and assigned.
Monitor urinalysis, breathalyzer, and other testing as assigned.

Qualifications

Master's degree required.
2 years of post-licensure experience preferred.
LISW, LCSW, LMHC, LPC, or LPCC required.

Requirements

Effectively communicate via written, verbal, in person and virtual methods.
Knowledge of community geographical awareness and of community and agency resources.
Skill in customer service techniques.
Skill in problem-solving.
Skill in consultation and networking.
