[Hiring] Recovery Coordinator @Change Grow Live

Role Description

An exciting role has become available for a Recovery Coordinator within our Intensive Support Team . The Intensive Support Team sits within our Severe Multiple Disadvantage (SMD) Programme, offering high level support for those that have additional barriers in accessing services.

With this role you’ll be working as part of our Intensive Support Provision, covering the south of Nottinghamshire. The team supports individuals who experience issues around drugs and/or alcohol, as well as developing wrap around support packages to ensure holistic approaches are taken to enable people to achieve the best possible outcomes.

• Manage a case load of service users who have been accepted via our Intensive Support Panel.
• Build relationships with and support individuals to access the support they need.
• Work in close partnership with our core and housing team, as well as external partners.
• Manage service user plans and risk.
• Advise service users regarding their substance/alcohol use, using recognised theoretical models (e.g. Motivational Interviewing).
• Work with Mental Health Agencies to ensure that service users have access to address their mental health issues.
• Work proactively to ensure that service users have access to ongoing support.
• Convening and contributing to service user case conferences.
• Work closely with all partner services, sharing appropriate information and ensuring that action plans are carried through.
• Represent the team at external forums where appropriate and requested to do so.
• Maintain accurate and comprehensive service user records in line with Change Grow Live's procedures and protocols and the requirements of the local authority.
• Participate in patterns of shift work to enable the team to maximise potential contact with service users, which will include working unsociable hours.

Qualifications

• Knowledge of substance misuse, alcohol misuse, and mental health.
• Understanding of service users who have multiple and complex needs.
• Understanding the importance of multi-agency working within a strength-based, asset-building approach and maintaining professional boundaries.
• Experience of facilitating multi-agency meetings.
• Experience of and a commitment to working positively in partnership with a range of statutory and voluntary agencies.
• Good administrative, recording, and reporting skills.
• Knowledge of safeguarding procedures applicable to children and vulnerable adults.
• Excellent communication, negotiation, and advocacy skills.
• Confidence to build and maintain relationships with a wide range of services.
• Confidence to work autonomously as well as part of a team.
• The ability to work with confidence and patience with clients unwilling to engage with services.
• Good level of IT literacy, including the ability to use IT systems, including Microsoft Office applications.
• Ability to critically reflect and evaluate interventions, service delivery, and areas for development.
• Full driving license and access to a roadworthy vehicle.
