[Hiring] Provider Support Specialist @Solari

Role Description

The Provider Support Services develop relationships with external Housing and Health Opportunities (H2O) providers to build and support the H2O network.

• Serve as the primary point of contact and subject matter expert on the H2O program for contracted providers.
• Manage a high volume of provider inquiries that require research, input, and follow-up to ensure accurate resolution and communication.
• Documents and maintains records relative to communications with assigned providers to ensure a mutual understanding of expectations, deliverables, and necessary follow-up.
• Plans and leads regular on-site and/or virtual meetings, educational seminars, etc. with contracted providers.
• Educates H2O providers regarding appropriate H2O program requirements, including best practices in homelessness services delivery.
• Collaborates with various departments or designees to develop training and education as required.
• Conducts ongoing review of provider performance and engages in program improvement conversations.
• Assists in care coordination for complex cases.
• Demonstrates ability to plan, organize, prioritize, and work independently and consistently with limited supervision.
• Must be comfortable working in the field with community partners while working independently and in an autonomous manner.
• Other duties as assigned.

Requirements

• 3-5 years of experience in Health, Human or Social Services, or related field, Preferred
• Bachelor's Degree in business, social work, or related field, Preferred
