Role Description
This role involves providing general supervision for Community Health Workers in your local community.
- Dedicate blocks of time for real-time telehealth visits with patients contacted by Community Health Workers.
- Evaluate patients' needs to determine eligibility for Community Health Integration services.
- Integrate Community Health Worker documentation app with your practice's Athena EHR instance.
- Review notes from Community Health Workers detailing the support provided to patients.
- Respond to Athena messages from Community Health Workers regarding urgent clinical issues.
- Work with a group of Community Health Workers sending 500-1000 new patients per year.
- Generate up to $330K from new billing and additional revenue for E/M visits.
- Complete required work in approximately 5-10 hours per week.
- Your practice will handle billing and collections, while IMPaCT Care pays local organizations employing Community Health Workers.

- Warm-hearted primary care physician.
Requirements
- Ability to conduct telehealth visits.
- Experience with Athena EHR system.
